What this site asks for, and what it never does.
How Chard Nutrition uses what you send through this website, why your health information stays out of it, and the rights UK GDPR gives you.
Example text for the Chard template, written for Chard Nutrition, an invented dietetic practice in London run by an invented registered dietitian. It is an example to be reviewed by a lawyer before you publish, and checked against your own practice software, insurer and professional body; nothing here is legal or medical advice.
1. Who is responsible
Chard Nutrition is the independent dietetic practice of Nell Avery, a registered dietitian. It is the controller of the personal data described here, under the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018.
Write or call: Chard Nutrition, Studio 4, 27 Halden Row, London E8 · 020 7946 0381 · hello@chardnutrition.example. Nell reads every request about your data herself.
2. What the contact form collects
The form on the Contact page asks for four things: your name, an email address to reply to, what you are writing about (first visit, follow-up, market walk or something else) and a short message. Nothing else is collected through it.
We use it only to answer you, and the lawful basis is our legitimate interest in replying to people who contact us, or the steps you ask us to take before booking. We don’t add you to a mailing list because you wrote to us.
3. Health information stays off this website
Information about your health is special category data under UK GDPR and needs extra protection. This website is not built to hold it, and it doesn’t ask for it: no diagnosis, medicine, weight, blood result or food diary goes through the site.
Please don’t put medical details in the contact form. If you do, we copy only what is needed into your clinical record when you become a client, or delete the message if you don’t.
Once you book, your health history is collected through the secure intake form of the practice software we use, under that provider’s security terms and the clinical privacy notice you receive before your first visit.
4. Booking, payment and video
The Book, Free 15-minute call, Intake form, Client login, video guide, free guide and review buttons open outside services chosen by the practice: online booking, payment and video calls. Anything you type there is handled by those providers under their own privacy policies, and they share with us only what is needed to run your appointment.
We never see or store your full card number.
5. Your clinical record
As a registered dietitian, Nell keeps a clinical record for each client, as the Health and Care Professions Council requires. It is processed for the provision of health care (UK GDPR Article 9(2)(h)), kept confidential and shared with your GP or another health professional only with your agreement, unless the law requires otherwise.
The clinical record is not part of this website.
6. Visit statistics
The site is built and hosted with Framer, which counts page views, devices and referring pages. We see totals only and cannot pick out one visitor. Where analytics need your consent, they run only after you accept them; the Cookie Policy has the detail.
7. Who else sees it, and where
Only Nell reads contact messages. They pass through Framer, which hosts the site and forwards form submissions, and the email provider of the practice. Some of these providers store data outside the UK; when they do, they rely on safeguards recognised under UK law, such as the UK International Data Transfer Agreement or an adequacy decision.
We never sell or rent your details, and we don’t share them with advertisers.
8. How long we keep it
A message that doesn’t lead to an appointment is deleted 12 months after our last reply. Clinical records are kept for the periods set out in UK health records guidance: for adults, usually 8 years after the last appointment, and for children, until their 25th birthday or longer.
9. Your rights
You can ask to see the personal data we hold about you, have it corrected or deleted, restrict or object to how it is used, and receive it in a portable format. We answer within one month. Write to the address above; there is no charge.
If you are unhappy with how we handled your data, you can complain to the Information Commissioner’s Office (ico.org.uk). We’d be grateful for the chance to put it right first.
10. Changes to this policy
When we change a tool or the way we use your data, this page changes and the date at the top with it. The version on this page is the one that applies.
